The printing is now landscape style.

Please make sure only one name is entered in this section. There
     is no limit in the Given by Section.

Please make sure the "Date Turned in or Mailed" line is filled in
     before you turn the form in. If a name fails to appear in the program,
     this will enable us to tell them when it was turned in.

Please make sure the person who is paying for luminary completes
      the information in the YOUR NAME, ADDRESS, and PHONE #
      section. We must have this if there is a question on the spelling of
      names or other information that if we need to contact them about.

A new form luminary checklist has been added this year. Please make sure you attach a checklist form to all luminaries turned in.

All luminary forms and money must be turned in no later than May 1, 2007 in order to be included in the tabloid that will be printed in the Sampson Independent.

The newspaper has given this deadline and will not accept any forms after this date. There will be less than a week to get all the names typed in, proofed, corrected, proofed, corrected and proofed again before the final printing.

In years past, in certain circumstances, a few have been accepted after the deadline. This year, May 1, 2007 is the final date. Any luminary turned in after this date will not be printed in the tabloid.

Luminaries need to be turned in at each team captains meeting in order for the typing of the names to begin as soon as possible.

Please make sure that names are readable and are spelled correctly on the forms. This will speed up the time that you have to stand in line when turning in your team's luminaries and will ensure that everyone will be happy with the spelling of their names.

You can continue selling luminaries after the May 1st deadline but they will not be printed in the newspaper. These will be placed on the track the night of the Relay but there will not be any kind of print out for these luminaries.
